If you would like to save a copy for your files or simply want to copy and paste it into you e-mail, follow these simple steps:

1. Click on Download (located just under the page - above the text)

2. Click on the document "Functions_Order_Form.doc"

3. You can either simply replace the text that is on the first page, or delete the first page entirely. 

4. Click on file then " Save or Save As" - Save the document under your prefered title.

5. Submit the form as an attachment or into your e-mail. Or via fax / telephone.
